What is CareSignal?

CareSignal is a new program to help people with certain conditions stay healthy from the comfort of home.

The program is geared towards Medicare and Medicare Advantage patients.

It is a FREE service with no additional costs to you.

You only need a phone to participate.

You will receive weekly text messages or phone calls from The Jackson Clinic asking about your condition-specific symptoms.

If you respond with worsening symptoms or health concerns, you can get help from a Jackson Clinic care coordinator.

The text messages or phone calls will arrive at a time that you choose, and it only takes a few minutes each week to respond.


Why should I enroll in CareSignal?

This is a great way to stay connected with your Jackson Clinic care team between doctor appointments.

Enrolling in the program will help you and your care team better manage your health.

The program is meant to provide peace of mind for people with the following conditions:

Hypertension (high blood pressure)

Diabetes (high blood sugar)

Congestive heart failure

COPD or other breathing difficulties

The information you provide is confidential, secure, and automatically added to your patient chart to be reviewed by your doctor.

Answering the weekly questions will help your doctor keep an eye on your condition without requiring you to leave home for frequent appointments.
