Appointments & RegistrationAppointmentsAppointments may be at The Jackson Clinic:
Appointments made in advance assure sufficient time will be reserved for the proper examination of each patient. We urge you to make appointments as far ahead as possible and to cancel appointments as far ahead as possible. Medical and surgical emergencies may cause delays; however, we do our best to keep appointments on schedule. If you need to be seen at The Clinic and have no appointment, we suggest a telephone call to the appointment desk as early in the day as possible. We will make every effort to schedule you with the physician of your choice. If this is not possible, care will be arranged through The Clinic's "on call" physician or at one of our Convenient Care locations. Appointments: (731) 422-0213 RegistrationUpon arrival at The Clinic, always register at the appropriate desk so the doctor will know you have arrived. If you have never seen a Jackson Clinic physician, a Clinic chart will be made that contains identification data (age, address, spouse's name, etc.), and will be used at all future visits. If you have never seen a Jackson Clinic physician in that specialty before, detailed patient information will be required. Please arrive at The Clinic 30 minutes before the appointment time if it is your first visit to The Clinic or the first time to see a Jackson Clinic physician of that specialty. If a Jackson Clinic physician either at The Clinic or at a local hospital has treated you, you will already have a Clinic account and chart. The Clinic should always have your current telephone number, address and insurance information. Please notify the Business Office when there are any changes to your information. It is important to tell the appointment secretary the reason for your visit so that he or she can allocate the proper amount of time for your appointment. Some of the different reasons for visits are complete history and physicals (new patient), surgical evaluations, specific purpose exams (life insurance, pre-employment, etc.), re-evaluations, history and physicals (established patients); re-check of previous treatment, and evaluation of a single current problem. A history is a review of past illnesses and any factors relating to your present health. The physical is a total examination of the body. The doctor may order laboratory tests and procedures. When necessary, consultations are arranged with specialists in other fields. Once the evaluation is complete, the doctor will discuss the diagnosis with you and recommend treatment as appropriate. If there is any question about the need for any of the above-mentioned procedures, please discuss them with your doctor before your exams begins. There may be minor illnesses which do not require such detailed examinations. Follow-up visits may also be required and they generally take less time for both doctor and patient. If you have been referred to The Clinic by your family physician, please inform our appointment secretary. Your physician will receive a report on your examination and our recommendations so that he may continue to treat you effectively. |
